  3. A statement under penalty of perjury that you have a good faith belief the material was removed or disabled due to a mistake or misidentification.

  4. Your name, address, phone number, and email address.

  5. A statement that you consent to the jurisdiction of the courts in your area and will accept service of process from the party who filed the original DMCA notice.

4. Repeat Infringers

We may terminate the accounts or access of users who are found to be repeat infringers of intellectual property rights.
